When is TSH measured?
TSH (thyrotropin) is a pituitary hormone that regulates the thyroid. A high TSH suggests an underactive thyroid and a low TSH an overactive one. The reference range at ages 15–59 is 0.5–4 mU/l, and the upper limit rises with age. According to the Current Care Guideline, hypothyroidism is diagnosed by measuring TSH and free T4.
The sample is best taken in the morning before 2 pm, because TSH varies with the time of day. If you take thyroxine, give the sample before the dose. High-dose biotin must be paused at least 8 hours before the sample. No fasting is needed.
TSH is measured for example for tiredness, weight change, feeling cold or palpitations, or to follow thyroid medication.
